COVID-19 Vaccine Requirement
All UW Professional & Continuing Education students, including fully online students, are required to be vaccinated against COVID-19 unless they receive a medical or religious exemption.
In summer 2021, students were asked to complete a student vaccination attestation form. But due to recent changes to Washington state’s vaccine requirements, you are now required to provide additional documentation for review by Hall Health Center – even if you previously completed the vaccination attestation.
All vaccinated UW students must now upload vaccination documentation, such as a vaccination card or other vaccination record. If you previously completed your attestation, simply upload the documentation. If you have not yet completed your attestation, you can do so when you upload your proof of vaccination.
For autumn quarter students, the deadline to upload your vaccination documentation is October 29, 2021.
Students Requesting an Exemption
In addition to the student attestation form, students who are requesting an exemption for medical reasons or deeply held religious beliefs must now complete an updated COVID-19 Medical or Religious Exemption Request. Philosophical exemptions are no longer allowed by Washington state.
For autumn quarter students, the deadline to submit an exemption request is October 29, 2021.
Autumn quarter students must complete the vaccine verification form or submit a request for an exemption is October 29, 2021. There will be a hold placed on your account for winter quarter registration if you haven’t completed one of these steps by November 5, 2021.
